On November 8, 2021, an employee had finished dipping a bundle of steel joists in a paint tank and had set it to dry on top of a stack of bundles using a 10-ton overhead crane. While the employee was walking to get the overhead crane's hook to move the next bundle, the bundle he had just moved fell, causing him to become caught between the bundle and the paint tank. The employee suffered a broken left femur and was hospitalized.

An employee was preparing to lift a 25,000-pound forging with an overhead crane using a spreader bar and a set of chains. The chains on one side of the forging snagged on set downs and would not slide into position. The employee used his right hand to position the chains to the center of the load when the crane began to lift the forging. The employee's right middle finger was caught between the chain and the forging, resulting in amputation to the first knuckle.

An employee was moving an aerial lift over a gantry crane track using an overhead crane. The employee was standing between a 74-inch steel pipe and the boom of the aerial lift when the final wheel of the lift cleared the gantry track and the boom swung over and pushed the employee into the pipe, resulting in fractured ribs and contusions on the left lung.

A mechanic was on top of the HHP in Bay 1 drilling out broken bolts. Another operator was running the overhead crane using a handheld remote, moving it from the south side of the HHP to the north side to pick up a fluid end. As the crane moved over the top of the HHP, it struck the mechanic, knocking him off the HHP onto the cement floor of the shop 13 feet below. The injured employee was hospitalized for broken ribs, a sprained wrist and stitches to the face.

While positioning the nylon sling to hoist a stack of 6"x12' aluminum round bars that was on top of 5"x12' rectangular flat bars, the round bars shifted, rolled back, and pinned the employee's left middle finger between a round bar and a flat bar, resulting in a fingertip amputation.

A temporary employee was moving a 2' x 20' steel beam weighting between 2500 & 3000 lbs. using an overhead crane. The employee sat the steel beam on stands with chains and lifted one end of the beam with the crane when a chain caught and pulled the beam off the stands. One end of the beam landed on the employee's left foot and ankle causing crushing injuries including broken bones that required hospitalization and surgery.

An employee was moving a steel coil that was 4.5" in width and weighed 1,340 lbs. from where it was being stored to the machine using an overhead crane. As the employee was attempting to roll the steel coil out from where it was stored to attach it to the crane, it started to become off balance and tipped over, landing on the employee's legs. The employee was trapped under the steel coil and sustained a broken tibia and fibula that required surgery.

On February 24, 2021, an employee was helping install a 250-pound metal pallet on a precast concrete form. The employee began moving dunnage below while the crane held the pallet about 12 inches above the ground. The pallet fell onto the employee's right ring and middle fingers, catching them against the dunnage and causing partial amputations to both fingers.

Two fingertips were amputated.

On August 31, 2020, an iron worker was installing a steel wall curtain while attached to the steel wall curtain approximately 12 feet from the ground. The steel wall curtain was on an overhead crane and employees were making ties at certain points. When the steel wall curtain was released by the crane it overturned and the iron worker was either struck by the steel wall curtain and/or struck an air compressor that was on the ground where he fell. The employee was hospitalized for fractured ribs.
